We provide a full array of required supports coordination services and the delivery of person-centered planning for persons with a wide array of disabilities in varied work and residential situations.
Our Supports Coordinators establish a close relationship with you, your family, care providers, and natural supports (friends, neighbors, etc.) and respond as appropriate to changes in your life.
We work with you to ensure the responsible delivery of service and goals along with family and others.
We are experienced and able to complete all of the assessments required by the MCPNs, Michigan Comprehensive Provider Networks. Our own Person Centered Profile has been commended by auditing bodies for its comprehensiveness and its focus on the person.
Your goals are developed and written according to your desires, wishes, health, safety, needs, and strengths.
Here are some of the ways we can help you:
Assisting in all matters of entitlements, such as Social Security, Medicaid, SSI, Medicare, Home Help Services, Section 8 housing, food stamps, wage records, spend-down, medical determinations, applications, and appeals.
Linking you with residential, social, recreational, respite, senior, transportation, medical, dental, and religious services.
Supports Coordinators understand and will assist you with employment, including self employment, individual job placement, group employment, and our own Resource Center based employment.
